Well, this is a very complex question and in order to give you an accurate answer I would need to know a few things. First off, if she is to practice medicine what field does she want to pursue. This will have a direct impact on how much free time she has. For example if she wants to do surgery or OB/Gyn then the residency programs are long and extremely time consuming. Surgery I believe is ~5 years and Ob/Gyn is ~4. The on call schedule is just as rough. Now if she is interested in say general medicine the residency requirements are significantly less.
Second, opening a privet practice is not only time consuming but is also highly stressful and again depending on the field may not even be possible. If Ob/Gyn then I would think with cost of insurance and time available to deliver babies at all hours of the day and week and to see enough office patients would be nearly impossible for a single doctor and still have free time.
Third, teaching is a noble cause and one can perform this duty along with continuing to practice medicine. Now again depending on the specialty your friend wants will depend on how much more time drain the teaching section will produce.
I am not sure the above really helps you out so here is a final little comment. Your friend will work it out and she will have free time but she has embarked on a never ending profession of learning and will require a lot of her time for the duration. Support her and she there will be more time after school and then again after residency.
